Cellular therapy products (CTPs) are a regulated product category covering any therapeutic product containing living cells intended to alter, modify, or replace biological function. The category encompasses: haematopoietic stem cell transplants (bone marrow/cord blood — the oldest CTPs), CAR-T cell therapies for haematological cancers (Kymriah, Yescarta — FDA/EMA approved), MSC-based products (Alofisel® for Crohn's fistulae — EMA approved), and a growing pipeline of iPSC-derived CTPs in early trials. Not all CTPs are stem cell products — CAR-T cells are genetically engineered T lymphocytes, not stem cells.
In the US, CTPs are regulated under 21 CFR Part 1271 (HCT/Ps) and, for more than minimally manipulated products, under the full BLA (Biologics License Application) pathway requiring Phase I–III clinical trials. In the EU, Advanced Therapy Medicinal Products (ATMPs) regulation governs CTPs — Alofisel® is the only MSC ATMP with EMA approval to date. In Turkey, the Ministry of Health's regenerative medicine framework permits GMP-certified MSC therapy under hospital exemptions, enabling clinical delivery of treatments not yet completed the full Western approval pathway.
The gap between scientific evidence and regulatory approval can be 10–20 years. MSC therapy has substantial peer-reviewed evidence across dozens of conditions but has not completed the full pharmaceutical approval pathway for most applications. This does not mean it is unsafe or ineffective — it means the commercial approval process is ongoing.
