A patient became the first person cured of type 1 diabetes with stem cell therapy. How reprogrammed stem cells produce insulin and what this means for millions with T1D.
Type 1 diabetes (T1D) is an autoimmune disease where CD4+ and CD8+ T lymphocytes destroy insulin-producing pancreatic beta cells. At diagnosis, most patients retain 15–40% of original beta cell mass — this "honeymoon phase" window, typically lasting 6–24 months, represents the optimal intervention period for MSC therapy. MSCs address T1D via three mechanisms: (1) immune reset — suppressing autoreactive T-cell populations to halt ongoing beta cell destruction; (2) beta cell protection — anti-apoptotic paracrine factors shielding remaining beta cells; (3) potential beta cell regeneration — promoting endogenous pancreatic progenitor cell activity and, in some settings, MSC trans-differentiation toward insulin-producing phenotype.
Published RCTs and controlled trials (Zhao et al. 2012, Hu et al. 2013, Carlsson et al. 2015) in new-onset T1D report: preservation of C-peptide (endogenous insulin marker) at significantly higher levels than controls at 1–2 years, reduction in exogenous insulin requirements (20–40% dose reduction in responders), and improved HbA1c. The honeymoon phase matters: patients treated within 12 months of T1D onset show 2–3× better C-peptide preservation than those treated later. For established T1D (>5 years, near-zero C-peptide), MSC therapy primarily reduces insulin resistance and HbA1c rather than restoring beta cell function.
Our T1D programme delivers 2–3 IV infusion sessions of WJ-MSCs combined with targeted pancreatic delivery via selective celiac artery infusion where appropriate. Pre-treatment evaluation includes C-peptide (stimulated and basal), HbA1c, anti-GAD/anti-IA2/anti-ZnT8 antibody titres, and T-cell subset profiling. Follow-up at 3, 6, and 12 months tracks C-peptide, insulin dose, and HbA1c with endocrinology co-management.
